Cefepime, a fourth-generation cephalosporin, is a broad-spectrum beta-lactam antibiotic. Its IUPAC name is (6R,7R)-7-[(2Z)-2-(2-amino-1,3-thiazol-4-yl)-2-methoxyiminoacetyl]amino-8-oxo-3-vinylenamino-5-thia-1-azabicyclo[4.2.0]oct-2-ene-2-carboxylic acid, with molecular formula C16H16N6O5S2 (molecular weight 408.44 g/mol). Structurally, it features a beta-lactam ring fused to a dihydrothiazine ring, with a methoxyimino group and a vinylenamino side chain, enhancing stability against beta-lactamases. Therapeutically, it treats respiratory, urinary, and intra-abdominal infections. Cefepime is water-soluble, with good tissue penetration. It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis.
