Cilostazol, IUPAC name 6-[4-(1-cyclohexyl-1H-tetrazol-5-yl)butyl]-3,4-dihydro-2-propyl-2H-1,2,4-benzothiadiazine 1,1-dioxide (C17H23N5O2S, molecular weight 365.47 g/mol), is a benzothiadiazine derivative with antiplatelet and vasodilatory properties. Structurally, it features a cyclohexyl group and a tetrazole ring. Therapeutically, it treats intermittent claudication via phosphodiesterase III inhibition, elevating cyclic AMP levels to promote vasodilation. It is a white to off-white solid (melting point 130โ133ยฐC), sparingly water-soluble but soluble in organic solvents. Its mechanism involves cAMP-mediated smooth muscle relaxation and platelet aggregation inhibition.
