Cinacalcet, IUPAC name (R)-4-((3-(4-chlorophenyl)-1-ethyl-1H-indazol-5-yl)methyl)pyridine-2(1H)-one, is a calcimimetic agent with molecular formula C20H18ClN5O (MW: 375.85 g/mol). Structurally, it features an indazole ring fused to a pyridine moiety, substituted with a 4-chlorophenyl group. Therapeutically, it treats hyperparathyroidism in chronic kidney disease and hypercalcemia in parathyroid carcinoma. It is a white to off-white solid, sparingly soluble in water (pKa ~5.5), with lipophilic properties. Cinacalcet allosterically activates calcium-sensing receptors (CaSR) in parathyroid glands, enhancing intracellular calcium signaling and suppressing parathyroid hormone (PTH) secretion.
