Clarithromycin, 6-O-methylerythronolide B (C38H69NO13, 747.86 g/mol), is a 14-membered macrolide antibiotic featuring a methyl-substituted lactone ring and cladinose sugar. It treats respiratory, skin, and Helicobacter pylori infections. A white crystalline powder with low water solubility and pKa ~7.8, it acts as a weak base. Clarithromycin inhibits bacterial protein synthesis by binding the 50S ribosomal subunit, blocking transpeptidation and mRNA translocation, thereby preventing pathogen proliferation.
