Desmopressin, IUPAC name 1-desamino-8-D-arginine vasopressin (C46H64N12O12S, molecular weight 1093.24 g/mol), is a synthetic cyclic nonapeptide analog of vasopressin. It belongs to the class of antidiuretic hormone (ADH) analogs, characterized by a D-arginine substitution at position 8 and absence of the N-terminal amine. Therapeutically, it treats diabetes insipidus, nocturnal enuresis, and von Willebrand disease. It exhibits high water solubility (25 mg/mL) and stability in acidic conditions. Mechanistically, it activates V2 receptors in renal collecting ducts, enhancing aquaporin-2 insertion to increase water reabsorption, with minimal vasoconstrictive activity due to V1 receptor selectivity.
