Drospirenone, IUPAC name 17ฮฑ-Spiro(2,4,6-trioxocyclohexane-1,3-dione-7,4'-pregn-1,4-diene-20-yne), is a synthetic progestin with molecular formula C24H29O5 (molecular weight 405.51 g/mol). Structurally, it features a spirocyclic ring fused to a pregnane core, containing ketone and spirocyclic oxygen functionalities. Classified as a selective progesterone receptor modulator (SPRM), it exhibits anti-mineralocorticoid activity via aldosterone antagonism. Therapeutically, it is used in combined oral contraceptives and for managing acne and hirsutism. Physicochemically, it is a white to off-white solid with low water solubility and high lipophilicity. Its mechanism involves binding to progesterone receptors, inhibiting ovulation, and modulating endometrial proliferation, alongside antagonizing aldosterone to reduce fluid retention.
