Esmolol, chemically 4-[[4-(1-hydroxy-2-hydroxyethyl)phenoxy]methyl]-1,2-benzenediol (C16H21O5, 293.33 g/mol), is a selective beta-1 adrenergic receptor antagonist. Structurally, it features a phenoxyacetamide core with a 3-hydroxy-2-hydroxyethyl side chain, enhancing metabolic stability. Therapeutically, it is employed for acute hypertension and arrhythmias. Its high water solubility (logP -0.5) and short half-life (~1โ2 hours) result from rapid ester hydrolysis. Mechanistically, Esmolol competitively inhibits beta-1 receptors in cardiac tissue, reducing heart rate and myocardial contractility via decreased intracellular cAMP.
